Soft tissue injuries need PEACE & LOVE

The rehabilitation of soft tissue injuries can be difficult. Over the years, acronyms guiding their management have evolved from ICE to RICE and PRICE to POLICE. Although widely known, the evidence for these treatments is limited. ICE/RICE/PRICE all focus on acute management with no guidance on how to manage the more subacute or chronic stages of tissue healing.

The British Medical Journal (BMJ) have proposed two new acronyms that can optimize recovery. The acronyms encompass the rehabilitation continuum from immediate care (PEACE) to subsequent management (LOVE). PEACE & LOVE underscores the importance of education and addressing psychosocial factors to enhance recovery. In addition, while anti-inflammatories show benefits on pain and function, the acronyms flag their potentially harmful effects on optimal tissue repair. It is suggested that they may not be included in the standard management of soft tissue injuries.

Whether you’re a seasoned athlete, a weekend warrior, or just dealing with the occasional mishap, here’s a simple guide to promote healing and recover with PEACE & LOVE:

Protection

Avoid activities and movements that increase pain during the first few days after injury.

ELEVATION

Elevate the injured limb higher than the heart as often as possible.

AVOID ANTI-INFLAMMATORIES

Avoid taking anti-inflammatory medications as they reduce tissue healing. Avoid icing.

C

COMPRESSION

Use elastic bandage or taping to reduce swelling.

EDUCATION

Your body knows best. Avoid unnecessary passive treatments and medical investigations and let nature play its role.

LOAD

Let pain guide your gradual return to normal activities. Your body will tell you when it’s safe to increase load.

O

OPTIMISM

Condition your brain for optimal recovery by being confident and positive.

VASCULARISATION

Choose pain-free cardiovascular activities to increase blood flow to repairing tissues.

EXERCISE

Restore mobility, strength and proprioception by adopting an active approach to recovery.
